OSHA DOL Card Replacement Policy
OSHA DOL card replacement is allowed if the student has a lost, stolen or damaged card. To qualify for a card replacement, the student must meet ALL the conditions below:
- Student has taken OSHA 10-Hour or OSHA 30-Hour training under 360training.com's providership.
- Student has successfully passed the OSHA 10-Hour or OSHA 30-Hour Course within 5 years.
- Student has never received an OSHA Replacement Card from 360training.com
Per OSHA regulations, the students do not qualify for a replacement card if they already received one.
Students can purchase the reprint online or by calling customer support. The support rep must process the payment and verify the following:
- Student's Completion credentials - Full name, completion date, physical address, phone number and email.
Once all details are verified, escalate the request to the OSHA reporting team and the card will be processed like any other DOL card request. All reprints purchased online through 360training will trigger an alert to the reporting team for processing the replacement.
The charges include standard mail, but for urgent shipment, an additional $24 must be charged.

There are two ways to get an OSHA card replacement:

- Contact Customer Support and submit your application over the phone.
NOTE: When you order your replacement online, you may be required to log in to your student account. Before you provide payment information, remember to edit your student profile and ensure the correct shipping address is associated with your account.
If you contact Customer Support, you’ll be asked to verify the following information:
- Full Name
- Completion Date
- Address on File
- Phone Number
You will also be asked about the training courses completed, such as whether the training was 10-hour or 30-hour. In addition, give us as much detail about how the replacement card was lost, damaged, or stolen, which will be useful in preventing identity theft.
Your billing address can be different. Please provide it when you enter your payment information because we will need to match it with the address on file with the credit card provider.
When you create your user profile, you can provide your shipping address, which should match the delivery address for where you want your DOL card shipped.
To ensure timely delivery of your DOL card, you can edit this at any time.
OSHA DOL card replacement costs vary by provider.
360training offers three shipping methods:
1. USPS Regular Mail
- Delivery Time: 5 -7 Business Days
- Cost: Included in cost of replacement
2. USPS Priority Mail
- Delivery Time: 2 – 3 Days (excluding Sundays)
- Cost: $20 per card
3. UPS Overnight
- Delivery Time: Next-Day (This will can be expected the day after it’s submitted to UPS.)
- Cost: $60 per card
NOTE: Any request received at any day of the current week will be processed and mailed the following week
Your official plastic, wallet-sized OSHA DOL card should ship within two (2) weeks following the completion of your course and survey. After the mandatory reporting process, the card will be printed and mailed via USPS First Class to the mailing address specified in your profile.
Upon successful course completion, you can access your training profile to either print or download the certificate of completion to your computer, smartphone, or tablet. While you wait for the arrival of your DOL card, you can use the certificate of completion as evidence that you've successfully finished the course.
Once you receive your OSHA DOL card, please verify receiving your card by phone within 24 hours of receipt of this letter.

- Follow the prompt to type in the number on the card you received.
IMPORTANT: To ensure your card isn’t delayed, please verify that your mailing address is listed correctly in your profile.
It is important to us that you receive your OSHA DOL card promptly. After completing he course, you will receive a temporary certificate of completion. Within two (2) weeks, your DOL wallet card will ship in the mail via USPS.
Please allow sufficient time for your card to reach you through the mail. The end-of-course survey must be submitted within 3 months of course completion, or a wallet card will not be issued.
If you do not receive it four (4) weeks after completing your course, please contact our customer support team. Wallet cards that the USPS can not deliver will be returned to our fulfillment center. If your card is returned, we will work with you to resend the card to the proper address. It is necessary to allow sufficient time for the card to be returned. If the USPS does not return it, the request will be processed as a replacement card and will be subject the associated purchase and delivery fees.
